目的 分析襄阳某医院8种呼吸道病原体的流行特征,为预防和诊疗急性呼吸道感染提供科学依据。方法 回顾性分析2021年1月—2024年12月就诊的急性呼吸道感染患者,采用胶体金检测患者血清甲型流感病毒(FluA)、呼吸道合胞病毒(RSV)、腺病毒(ADV)、人副流感病毒(HPIV)、肺炎支原体(Mp)、肺炎衣原体(Cp)、柯萨奇病毒B组(CVB)和埃可病毒(ECHO)8种病原体的IgM抗体,病原体IgM抗体阳性判断为该病原体感染。结果 阳性检出率平均5.45%(1449/26 954),以2023年阳性检出率最高,为8.2%。检出率排前三的病原体为FluA(12.22%,451/3690)、Mp(9.63%,638/6 627)、Cp(2.96%,85/2 827)。男性以FluA、Mp、Cp和RSV流行为主,女性以FluA和Mp流行为主。未成年患者以FluA和Mp流行为主,19~60岁患者与未成年患者流行的主要病原体一致,60岁以上患者8种病原体流行趋势不明显。结论 襄阳地区急性呼吸道病原体有一定的流行特点和趋势,可为临床诊疗提供参考,以提升医生对急性呼吸道感染的诊疗效率。
Abstract:ObjectiveTo analyze the epidemiological characteristics of 8 respiratory pathogens in a hospital in Xiangyang, and to provide scientific basis for the prevention and diagnosis of acute respiratory infections.Methods A retrospective analysis was conducted on patients with acute respiratory infections who visited the hospital from January 2021 to December 2024. The IgM antibodies of 8 pathogens, including influenza A virus(FluA), respiratory syncytial virus(RSV), adenovirus(ADV), human parainfluenza virus(HPIV), Mycoplasma pneumoniae(Mp), Chlamydia pneumoniae(Cp), group B Coxsackievirus(CVB), and Echovirus(ECHO), in the patients' serum were detected using colloidal gold. A positive result for the IgM antibody of a pathogen was defined as an infection with that pathogen. Results The average positive detection rate was 5.45%(1,449/26,954), with the highest positive detection rate in 2023 at 8.2%. The top three detected pathogens were FluA(12.22%, 451/3,690), Mp(9.63%, 638/6,627), and Cp(2.96%, 85/2,827). Males were mainly infected by FluA,Mp, Cp, and RSV, while females were mainly infected by FluA and Mp. Adolescents had the highest prevalence of FluA and Mp, while patients aged 19-60 had the same main pathogens as the adolescents, and the prevalence trends of the 8 pathogens were not obvious in patients over 60 years old. Conclusion There are certain epidemiological characteristics and trends of respiratory pathogens in Xiangyang region, which can provide a reference for clinical diagnosis and treatment, and improve the efficiency of doctors in diagnosing and treating acute respiratory infections.
